Our client is a sufficiently ran and operated new build nursing home based in Limerick City. They are looking for a Gerontology nurse to join them on a full time perm basis. Flexibility is offered re shifts.

The successful candidate will be providing care and support to Elderly Patients focusing on their physical, Emotional and psychological needs.

Responsibilities in the role

Assessment and Care Planning:

Conduct thorough assessments of elderly patients to identify their medical conditions, functional limitations, cognitive status, and psychosocial needs.

Develop individualized care plans that address the specific needs and goals of each patient, taking into account their medical history, current health status, and preferences.

Medication Management:

Administer medications as prescribed by physicians, adhering to proper dosage and scheduling guidelines.

Monitor medication effectiveness and potential side effects, adjusting treatment plans as necessary.

Chronic Disease Management:

Manage chronic illnesses commonly associated with aging, such as diabetes, hypertension, and heart disease.

Educate patients and their families about disease management, lifestyle modifications, and self-care techniques.

Mobility and Rehabilitation:

Implement mobility and exercise programs to maintain or improve patients’ physical strength, flexibility, and overall functional status.

Collaborate with physical and occupational therapists to design and execute rehabilitation plans.

Pain Management:

Assess and manage pain in elderly patients, considering their unique pain perceptions and potential comorbidities.

Utilize non-pharmacological interventions, in addition to medication, to alleviate pain and enhance comfort.

Psychosocial Support:

Provide emotional support and mental health care to elderly patients who may experience loneliness, depression, anxiety, or cognitive decline.

Foster a supportive environment to promote social engagement and mental well-being.

End-of-Life Care:

Collaborate with interdisciplinary teams to ensure appropriate palliative and end-of-life care for patients nearing the end of their lives.

Offer emotional support to both patients and their families during this challenging time.

Documentation:

Maintain accurate and detailed electronic health records, documenting patient assessments, interventions, and outcomes.

Ensure compliance with legal and regulatory documentation requirements.
